How Nicotine Replacement Therapy Works, and Why It Gives You a Real Advantage

What NRT Actually Does

There’s a version of quitting smoking that gets a lot of cultural credit: going cold turkey. No aids, no support, just deciding to stop and stopping.

It sounds strong. The data, however, tells a different story.

Nicotine replacement therapy (NRT) is a medically recognized approach to smoking cessation that works by delivering a low, controlled dose of nicotine to the body  without the thousands of harmful substances found in tobacco smoke. No tar. No carbon monoxide. None of the carcinogens are directly linked to lung cancer and heart disease.

The goal of NRT isn’t to keep you dependent on nicotine forever. It’s to reduce the intensity of physical withdrawal symptoms and cravings  giving your body a managed, lower-level dose of nicotine while you focus on breaking the behavioural and psychological habits around smoking. It addresses one side of addiction at a time, which turns out to be a much more effective approach.

What the Research Shows

NRT is among the most extensively studied smoking cessation interventions in medicine. Cochrane Reviews, widely considered the global benchmark for systematic evidence, have consistently found that nicotine replacement therapy significantly increases the likelihood of quitting successfully compared to attempting to quit without any support.

That benefit is further improved when NRT is used alongside a structured plan and behavioural support. The evidence is clear: using a quit aid as part of a comprehensive smoking cessation program produces better outcomes than NRT alone, or willpower alone.

Health Canada has authorized NRT products under the Natural Health Products Regulations as stop smoking aids that can help reduce withdrawal symptoms, including nicotine cravings associated with quitting smoking.

Health Canada-Approved NRT Formats

Several NRT formats are available without a prescription at pharmacies across Canada  each approved by Health Canada as a stop smoking aid for adults 18 and older.

Nicotine Gum (2mg or 4mg) A temporary aid to help stop smoking cigarettes or break the cigarette habit, when used as part of a smoking cessation program. Nicotine is released gradually through a specific chew-and-park technique. Up to 20 pieces per day.

Nicotine Mini Lozenge (1mg or 2mg) A stop smoking aid that can help reduce withdrawal symptoms, including nicotine cravings associated with quitting smoking, when used as part of a comprehensive smoking cessation program. The lozenge dissolves slowly in the mouth for approximately 10 minutes. Important: it should not be chewed or swallowed. Follows a structured 12-week programme with a gradual taper in frequency.

Nicotine Liquid Spray (1mg or 2mg) An instant release mouth spray  and the fastest form of nicotine replacement therapy. Works on symptoms within 60 seconds. Used when you crave a cigarette or are experiencing nicotine withdrawal symptoms. Available in 1mg (up to 64 sprays per day) and 2mg (up to 32 sprays per day).

NRT Is a Tool, Not a Crutch

The most common hesitation people have about NRT is that it “still contains nicotine.” But this misidentifies what quitting smoking is actually about.

The harm from smoking doesn’t come from nicotine alone. It comes from combustion  from inhaling tobacco smoke and the thousands of toxic compounds it produces. NRT eliminates that exposure completely, delivering nicotine in a dramatically safer form while your body and brain gradually adjust.
